Health insurance requires patients to choose a preferred doctor. The attending physician is generally a general practitioner, but may also be a specialist. He or she will be responsible for your medical follow-up, coordinating your care and referring you to a specialist if necessary.
By consulting your treating physician, or a specialist he or she has recommended, you are respecting the so-called coordinated care pathwayand you'll be better reimbursed.
The declaration of choice of treating physician can be made by online if the doctor suggests it and you agree.
In practice, during an office consultation and upon presentation of your carte VitaleThe doctor you have chosen as your primary care physician completes the declaration online and sends it directly to your health insurance fund.
If your doctor cannot carry out this operation online, you can send the form of "Déclaration de choix du médecin traitant" (form S3704) to your health insurance fund.
This form can be downloaded from the ameli.fr or can be requested directly from your doctor.
It must be completed with your doctorand signed by the doctor and yourself.
You will then need tomail to your health insurance fund.
Attention
If the patient does not have an attending physician and therefore does not follow the coordinated care pathway, higher rates will be applied or reimbursements will be reduced.
